qTip modal accept/decline form

I'm making a utility for work and one of the requests was to ask why the user was trying to access what they clicked on. I was looking to do a modal that would have a drop down of reasons that the user would select. If they hit continue it would take them to the page requested, or they can hit decline and it would go back to the previous page. I tried searching and couldn't find anything that would start me in the right direction.

Any ideas if this is possible, and would anyone be willing to help me through it??
